
Brian Zukowski, P.T., D.P.T.
Dr. Zukowski graduated with academic distinction from the University of Arizona with his Bachelor’s in Health Sciences and later from Northern Arizona University with his Doctorate in Physical Therapy. While completing his graduate studies, Brian’s passion for preventative medicine and athletic competition lead him to become the lead research assistant in a study examining preventative neck bracing in extreme motor sports. He is currently an active member of the American Physical Therapy Association and Sports Physical Therapy Section.
Brian enjoys working with clients looking to manage and resolve orthopedic conditions and athletes from all levels of competition. Brian strives to help his clients become proactive about their health and return to their optimal level of function with individualized treatment programs. Beyond orthopedics, prior clinical experiences include working with patients in the emergency department and acute care setting.
In his free time, Brian enjoys cycling, hiking, and practically any sporting competition. Jon P. Davison, PT
Jon received his undergraduate and Physical Therapy degree from the University of Evansville, Indiana in 1997. Jon has worked in busy orthopedic clinics in Bend, Oregon and Tucson. He has studied with both the North American Institute of Orthopedic Manual Therapy and the International Acadamy of Orthopedic Medicine. He helped establish the Emergency Department Physical Therapy Program at St. Joseph's Hospital in Tucson and later the Sports Plus Physical Therapy Clinic for Carondelet Health Network. He joined ProActive in April of 2005. Jon has experience with training, treating and teaching athletes from all levels of competition. He enjoys a good bike ride and soccer match.

Carolyn Adler, PT, DPT
Dr. Adler received her bachelor’s degree in Physiological Sciences from the University of Arizona in 2004. She later graduated with academic distinction fromNorthern Arizona University in 2007, receiving her Doctorate of Physical Therapy as well as her graduate Certificate in Disability Policy and Practice through theInstitute of Human Development at NAU. Carolyn has been an active memberof the American Physical Therapy Association since 2004, and has had thehonor of presenting at the Arizona APTA’s Orthopedic Update meeting. She isa lifelong learner, and has studied with the International Academy of Orthopedic Medicine since 2007.
Carolyn’s interest in physical therapy stems from a strong background in theperforming arts, including dance, singing, and piano performance. She enjoysworking with patients of all ages and backgrounds, including athletes, performing artists, active adults, expectant mothers, geriatric population, and those dealing with movement & gait disorders. She is passionate and enthusiastic abouthelping others achieve their goals and return to a healthy, active lifestyle. When not in the clinic, Carolyn enjoys spending time with her husband, Jason, and their two children, Nathan & Andrea. You can find her running on the River path, singing with Arizona Repertory Singers, or involved in music ministry at the Newman Center. Carolyn Received Her Bachelor’s Degree In Physiological Sciences From The University Of Arizona In 2004.

Kevin Fay PT, DPT, CSCS
Kevin earned his Doctor of Physical Therapy Degree from A.T. Still University in Mesa, Arizona in 2004. Prior to becoming a physical therapist, he graduated from the University of Dallas with a Bachelor's Degree in Biology and spent time working in the health and fitness industry as a certified personal trainer and a physical therapy technician.
Through his work and clinical internship experience Kevin has worked with a wide variety of clientele, ranging from professional athletes to the deconditioned. Along with being a PT, Kevin is also a Certified Strength and Conditioning Specialist through the National Strength & Conditioning Association. He enjoys integrating important concepts of rehabilitation with physical fitness and athletic performance into his patient's treatment programs.
He is committed to furthering his education and clinical skills and attends continuing education seminars, workshops and conferences numerous times each year. He specializes in the evaluation, treatment and prevention of orthopedic and sports related injuries.
Kevin is a native of Arizona and has worked primarily in the Phoenix area over the past 5+ years. As a physical therapist, he thoroughly enjoys improving the lives of the patients he works with everyday at ProActive Physical Therapy. Kevin is happily married and has 2 beautiful children.

John Woolf, MS, PT, ATC
A graduate of the Northern Arizona University College of Physical Therapy. He has a masters degree in Exercise and Sports Science and Athletic Trainer Certification from the University of Arizona. John is the former Director of Sports Medicine for The University of Arizona where he coordinated rehabilitation and medical coverage for division I athletes. He belongs to the American Physical Therapy Association and the National Athletic Trainers' Association. John has lectured in the University of Arizona's Sports Medicine Fellowship Program and continues to lecture nationally on the topics of diagnosis and management of musculoskeletal pain and the application of a biopsychosocial approach to health care. He is the co-director of the International Academy of Orthopedic Medicine-US(http://www.iaom-us.com/). In his spare time John races bicycles in the Tucson community, enjoys his wife, Chris and two children, Natalie and Timber.

Alicia Allie, DPT
Alicia received a Bachelor's degree in Biomedical Sciences from Western Michigan University, where she was on full athletic scholarship as a member of the varsity gymnastics team. She later graduated with academic distinction from Northern Arizona University with her Doctorate of Physical Therapy. She is an active member of the American Physical Therapy Association including the Orthopedic Physical Therapy and Hand Therapy sections. She has a passion for treating the upper extremity as well as all other orthopedic and sport related conditions, and is working towards her goal of becoming a Certified Hand Therapist. In her free time Alicia enjoys running and cycling with her husband.

Ben Shook, DPT
Dr. Shook graduated from Texas Tech University Health Sciences Center with a Doctorate of Physical Therapy (DPT) as well as a Bachelor's in Health Sciences. He is completing his certification with the International Academy of Orthopedic Medicine (IAOM) to specialize both in the treatment of the spine and extremities. Providing diagnosis specific treatment plans for his patients is paramount in his treatment approach. As a former collegiate athlete, Ben is able to provide insight not only to the competitive athlete, but also to the weekend warrior who wants to achieve his or her personal goals. Ben also has an interest in pain management interventions for the patient that requires more than physical therapy and would benefit from a multidisciplinary approach to healthcare. Outside of the clinic, Ben enjoys exploring the outdoors with plenty of hiking and running on trails that Tucson has to offer.
Ilene Kanefsky, BS, LMT, CLT
Ilene has been a practicing massage therapist for over 21 years. She was originally licensed in the state of Florida in 1990. She then decided to return to school and graduated magna cum laude in cardio-respiratory therapy from Tennessee State University in 1997. Ilene’s first passion called her back and she has been a full time practitioner ever since her graduation.
She is certified in Neuromuscular Massage and Manual Lymphatic Drainage.
Ilene is passionate about being a part of her clients' health care team and helping them lead a healthy life!
Ilene says, "I am happy to be back in Tucson with its beautiful mountains and sunsets . When I am not in school or working I enjoy taking advantage of the hiking and camping that the Tucson area affords."

Frank Tomizuka, CM
Frank graduated from The Institute for Physical Fitness and Myotherapy. He is a licensed massage therapist and belongs to the International Myotherapy Association. Frank has been a student of mindfulness meditation for 8 years. One of the main focuses of his practice has been to increase understanding of the relationship between physical pain, thought and emotion.He brings his personal insights to work effectively and compassionately with people who are experiencing chronic pain. Frank also directs networking and information technology for Proactive Physical Therapy and Integrative Pain Center of Arizona .

Albert Galvan, PT Technician
Albert Graduated from Apollo College in 2005 with a Diploma as a Physical Therapist Technician. Born and raised in Tucson, He attended Pueblo High School Class of 1972 and, for four years, was a three sport athlete for the Warriors. He married his wonderful and lovely wife Rosana and they have two grown children, Anthony (Andrea) and Lena. He also has a grandson, Aydan, who is two years old. Albert enjoys helping patients get back to where they were before their injury with a lot of heart and compassion. He enjoys staying physically fit with jogging, weight training and hiking up Tumamoc Hill four times a week. His favorite saying is "Don't let this gray hair fool ya!!" He is also a die hard Washington Redskins Fan!!!
Nancy Simmons MS, ATC
Nancy (PTA - CA only) Graduated from University of Oregon in 1985 with a Masters in Adaptive PE and received her NATA certification that same year. Was licensed as a PTA in the state of California in 1988. Has worked in Physical Therapy Clinics, hospitals, and high schools in Oregon, California, and since 1991, Arizona. Currently is head athletic trainer at Catalina Foothills High School providing coverage for athletes in all sports.
Sylvia Rodarte, PT Tech
Sylvia is a Native of Nogales Arizona. She graduated from the Apollo College PT Tech program in October 2009 with a 4.0 GPA. She completed her externship with ProActive Physical Therapy in July & August 2009. She has been a full time employee working at all 3 ProActive locations since finishing her externship. She is the only bi-lingual employee at the central clinic and assists in all treatments for our spanish speaking patients. The thing she enjoys most about working for ProActive is helping people become healthier. In her spare time she enjoys exercising, dancing, biking, hiking, and being ProActive's "fashionista."
